During implementation projects, the CSI Technical Team provides clients with guidance and support, from Advisory Services and Project Management to on-the-ground resources needed to conduct various project efforts. CSI is proud to employ a team of industry veterans comprising prior Epic leaders and long-time strategic technical experts from some of the top medical facilities in the country. I was fortunate to have time to meet with two of these colleagues to discuss their solution offerings and share some insights.<\/p>

One of CSI Companies\u2019 top technical service offerings is the Clinical Chart Abstraction solution. With over 15 years of EHR implementation experience, our technical team has truly fine-tuned this offering, resulting in top KLAS ratings year over year. The customized solution offering ties into any type of facility transition. Whether a full EHR install, a Community Connect extension, or new facility acquisition, CSI has a long successful history of supporting these efforts, and a best practices model worth sharing.<\/p>

To define the activity, Clinical Chart Abstraction is the effort of manually reviewing and transcribing patients\u2019 clinical data, as selected by the facility. These data points range from allergies to medications to immunizations and vary based on the client\u2019s needs. There are dozens of data points to consider. The information is extracted from the legacy EHR and will be entered into the new EHR. As clinicians will work from the new system after go-live, the Abstraction project ensures the patient data from the old EHR is fully available to them at go-live.<\/p>

Ana Lagunez, Vice President of Technical and Strategic Services, explains, \u201cthe goal is to quickly and accurately validate and enter clinical data into the new EHR so that at go-live, providers and clinicians can easily view scheduled patients\u2019 charts.\u201d<\/p>

We discussed with Ana why CSI continues to be so successful in delivering abstraction services. What do you think differentiates CSI from other vendors?<\/h5>

\u201cWe have extremely high quality assurance and productivity standards. We\u2019ve worked incredibly hard to build a highly trained team of expert abstractors and quality assurance leads.<\/p>

\u201cWe\u2019ve grown the strongest and most proactive team of experts in the industry.\u00a0 In the past 5 years, we\u2019ve directed more than 200 abstractors with both clinical and EHR experience. They all undergo rigorous testing, training and quality assurance checks to ensure work always meets strict accuracy and productivity standards.<\/p>

\u201cCSI partners with clients to holistically strategize and steer each project, especially when weighing clinical end user impacts or encountering unexpected challenges. Our tech-savvy leads also complete hands-on technical configuration to create the most efficient workflows with transparent reporting.\u00a0 We accomplish far more than staff augmentation; we advise our clients as co-leaders to guarantee success.\u201d<\/p>

How do we measure success?<\/h5>

\u201cWe are entirely data-driven, so we use real-time productivity and accuracy tracking to measure our success. At the start of each project, we carefully test and calculate the expected number of minutes to abstract each chart and gauge the range of data complexity across various patient types to know exactly how to sequence the work and load-balance our team. Our active monitoring ensures that we stay on-track and within budget.<\/p>

\u201cWe confirm end user satisfaction by validating specific data entry workflows and protocols before starting, then continue to receive direct feedback from clinical leads throughout go-live.<\/p>

Our track record of successes with increasingly large and complex projects (which still come in on time and under budget) are a testament to how well refined our best practices and methodologies are.\u201d<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Ana continued with, \u201cIncorrect initial scoping is common, which is why our early involvement is so critical.\u00a0 Clients may not have accurate appointment or order volumes, nor a gauge on the impact for internal staff\u2019s workloads when certain data elements aren\u2019t abstracted. CSI attempts to carefully assist in comprehensive scoping, but if we are not brought in until the eleventh hour, we often have to support sudden scope increases. Luckily, we are an agile and seasoned team that can quickly pivot.<\/p>

\u201cAnother common risk is not allotting enough internal staff to prepare. This is especially critical for entering future orders and referrals, which must be extracted into reports that are generated multiple times leading up to go-live, so CSI can enter only active orders into the new EHR for go-live. The reports can be very difficult to generate as it requires extensive mapping between the legacy and new EHR\u2019s order names, IDs and order-specific questions. With our team\u2019s technical EHR and clinical expertise, we help to translate data between EHRs. We expedite the process with detailed design specifications and thorough testing.<\/p>

\u201cOther risks that often crop up are delayed CCD loads, erroneous order extracts or incomplete inventories of departments. CSI attempts to mitigate these risks by incorporating lessons previous clients learned into our project plans, and allotting buffer time to prepare and begin the project.\u201d<\/p>

Ana, at what point in an EHR implementation does planning usually begin for Abstraction, and why is this important?<\/h5>

\u201cWe typically reach out to clients 6 months before go-live to confirm whether an abstraction vendor will wholly own or partially support internal abstraction efforts. Three months pre-live, we finalize scope and contracts, then complete technical preparation. The actual abstraction usually starts at least 1 month before go-live then continues on a rolling basis to ensure newly scheduled patient charts are abstracted.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Another top-rated solution from CSI Companies is our Technical Dress Rehearsal (TDR) offering.\u00a0 This phase of an EHR implementation requires teams to be deployed to test workstations and equipment in preparation for go-live. The specific tests for each device are dictated by the type of department in which the workstations live. The goal is to confirm that at go-live, each workstation will have access to the necessary EHR functionality for its respective department, or hospital area, and be able to successfully print and\/or provide its function. In addition to workstations, TDR teams are also testing peripheral items, such as Business Continuity Access (BCA) machines, scanners, and e-signature pads.<\/p>

Marcie Perkins, Director of Technical Services, explains, \u201cTechnical Dress Rehearsal is a critical phase in the implementation of a new EHR.\u00a0 This is where all the hardware devices like computers, printers, and scanners are thoroughly tested to ensure they function correctly before the system goes live. CSI positions the efforts as a practice run to ensure the identification of any potential technical issues with the equipment and the resolution of those issues.\u201d<\/p>

Marcie, why are healthcare facilities engaging CSI for TDR efforts?<\/h5>

\u201cEngaging CSI means that our customer\u2019s application analysts, and other project team members, can continue to focus on system support and operational readiness during the most time-critical stage of the project, while a team of TDR-experienced professionals ensures that all devices are tested for connectivity, print routing, and functionality.\u201d<\/p>

How does CSI work with clients in identifying the scope of their TDR efforts and the actual equipment needing to be tested?<\/h5>

\u201cAt CSI, we take a customized approach to Technical Dress Rehearsal (TDR) by collaborating closely with healthcare clients to define their unique scope and testing needs. Whether it\u2019s a small clinic, a regional hospital, or a multi-site healthcare system, we tailor our strategy to match the complexity of the environment. \u201cMy team also customizes the testing approach based on the size and operational structure of each facility. For smaller clinics, we focus on key workflow-critical devices to minimize downtime, while for large health systems, we implement a phased, site-by-site strategy to ensure comprehensive validation across multiple locations. Through real-world scenario simulations, we test how equipment interacts within various clinical environments, helping to optimize workflows, improve user adoption, and reduce post-go-live issues. This proactive, risk-mitigation approach not only prevents costly disruptions but also enhances patient safety, clinician efficiency, and overall system performance.\u201d<\/p>

You\u2019ve spent years defining this solution offering, can you tell me a bit more about the team itself, and why customers are benefiting from using CSI resources?<\/h5>

\u201cCSI provides a team of seasoned technical resources who have extensive experience working in a healthcare setting around patient care and possess in-depth knowledge of the various Epic or other EHR modules. They have an advanced understanding of workflows, and technical knowledge to identify, triage, and resolve issues more effectively. Our teams collaborate with client resources to assist in planning, coordination, and execution, proactively identifying and resolving technical and operational issues. By ensuring systems are fully functional and workflows are optimized before go-live, the team mitigates risks, enhances clinician confidence, and prevents costly post-launch disruptions.\u201d<\/p>

Given our continued success in Technical Services, what are some ways you monitor a project and what are metrics we provide to our clients?<\/h5>

\u201cCSI\u2019s proven best practices have consistently resulted in device pass rates exceeding 95% within the planned TDR schedule and will allow for successful and timely completion prior to go-live. In addition, CSI tracks the identification, mitigation, and resolution of risks and issues, ensuring they are addressed proactively before impacting the project. What are some risks or things to watch out for during this phase of an install?<\/h5>

\u201cThe biggest risks in a TDR project often stem from both client requirements and responsibilities that, if not carefully managed, can significantly impact the success of the initiative. One key risk is the use of local staff who lack experience in TDR to save on travel costs. While this may reduce expenses, it can lead to inefficiencies and errors due to the lack of expertise. Similarly, relying on internal staff to manage the project, especially when competing priorities arise, presents a risk of delays or insufficient focus on the TDR efforts. Assigning internal staff as Team Leads or Coordinators without considering these competing demands may further exacerbate this issue.<\/p>

\u201cOutdated or poorly maintained hardware inventory is another major risk, as devices that don\u2019t meet Epic upgrade standards may require costly upgrades or replacements. An insufficient test script library can lead to incomplete or ineffective testing, while a lack of a robust change management strategy or an underdeveloped communication plan can create confusion and impede stakeholder alignment. Without frequent and targeted updates to impacted leadership and management, there\u2019s a risk of miscommunication, leading to misaligned expectations and delays. Addressing these risks is critical to ensuring smooth project execution and successful outcomes.\u201d<\/p>
